This should reach the DVLA within 4 months of completion of the consultation. This needs to be renewed every 5 years until you are 65 years old and every year after this.
Any fully qualified doctor who has completed one of these assessments before and who has relevant experience in the medical industry.
One of the following is needed:
A letter of evidence showing regular diabetes check-ups must be provided alongside proof that you are effectively managing your condition. Also, evidence of a blood sugar monitor that shows the last 3-months of your checks must be provided. If you are taking certain tablets such as Sulphonylurea or Glinide, you should monitor your blood sugar twice daily at an appropriate time relevant to driving - ie two hours before and every two hours with driving. You must have a clear understanding of diabetes and your condition before visiting us.
You should rest for at least for 6 weeks and you may require a treadmill test before driving.
Simply go to your GP and see them, they can print one off.
We take your blood pressure and monitor your pulse, if it is high, you may need additional readings. An eye test with and without glasses is also carried out and we also measure your weight and height. During your consultation, we ask questions about your previous medical history and test you for any potential signs of diabetes.
