What If I Take Medication?

Our doctors need to know what medication you are taking and in what dosage, so please ensure you bring all medication to your consultation.

Where Can I Get the D4 Medical Form from?

You can get the D4 medical form here

How Long Is It Valid For?

This should reach the DVLA within 4 months of completion of the consultation. This needs to be renewed every 5 years until you are 65 years old and every year after this.

Who Can Complete the D4 Form?

Any fully qualified doctor who has completed one of these assessments before and who has relevant experience in the medical industry.

What ID Should I Bring with Me?

One of the following is needed: 

  • Passport
  • Driving Licence
  • Birth Certificate
  • Medical Card
  • Bank or Building Society Statement
  • National Insurance Card
  • Payslip
  • Benefit Book or Letter from Benefit Agency

What If I Have Diabetes or Use Insulin?

A letter of evidence showing regular diabetes check-ups must be provided alongside proof that you are effectively managing your condition. Also, evidence of a blood sugar monitor that shows the last 3-months of your checks must be provided. If you are taking certain tablets such as Sulphonylurea or Glinide, you should monitor your blood sugar twice daily at an appropriate time relevant to driving - ie two hours before and every two hours with driving. You must have a clear understanding of diabetes and your condition before visiting us.

What If I Have a Heart Attack?

You should rest for at least for 6 weeks and you may require a treadmill test before driving.

How Would I Get a Record of My GP Summary?

Simply go to your GP and see them, they can print one off.

What Happens During Your Consultation?

We take your blood pressure and monitor your pulse, if it is high, you may need additional readings. An eye test with and without glasses is also carried out and we also measure your weight and height. During your consultation, we ask questions about your previous medical history and test you for any potential signs of diabetes.

How Long Does the Consultation Last?

The consultation can last between 15 to 20 minutes depending your medical history.
